Louisiana Small Town Health Professional Tax Credit
The Small Town Health Professionals Credit provides and individual income tax for certified medical primary care health professionals. Applications will be approved as eligible for the credit on a first-come, first served basis as determined by the postmarked or received date of a completed Louisiana Small Town Health Professionals Credit Application. No taxpayer shall receive a credit for more than five years.
The Details:
Who: Rural primary care practitioners, including: Family Physicians, General practice physicians, Internal Medicine, Pediatrics, Obstetrics/Gynecology physicians, primary care Nurses practitioners, primary care Physician Assistants, Dentists and optometrists.
What: The Small Town Health Professionals credit allows approved applicants to receive a nonrefundable tax credit in the mount of $3,600 unless subject to proration
When: For 2019, this program accepts applications from September 1st- October 31st. No late applications will be accepted.
Where: Must practice in a rural area as defined in rules promulgated by the Department of Health and be located in a Health Professional Shortage (HPSA) designated area. Please visit: https://www.ruralhealthinfo.org/am-i-rural to determine whether the practice location is in a Rural and HPSA area.

- Last 4 digits of your Social Security Number
- Scanned copy of your current medical license
- Medical License Number
- NPI Number
- Medicaid/Medicare Numbers
- Saved “Am I Rural” report (#2 below)
- Patient Information
- How many patients to you examine daily/annually
- Total # of Medicaid Patients
- Total # of Medicare Patients
- Total Number of Uninsured Patients
- Total Number of Sliding Fee Scale/Indigent Patients
